Request access online

Use the secure request form. You will verify your email, and the office will confirm your identity before sending an invitation.

Call the office

Call the front desk and ask for patient portal access. Staff will verify your information and begin the invitation.

Ask during your visit

Tell a staff member that you would like portal access while you are at the office. They can start the verified request with you.

For your privacy, portal access is never granted automatically. Do not send private health information by ordinary email.
